1/ Application, purple
2/ declaration of living together, 1 written by boyfriend, 1 written by girlfirend. All in Czech.
3/ 1st year and 2nd year Original Rent agreement, with your name and your boyfriend's name on it
4/ Evidence of being in relationship, photos for a year or more, flight ticket, anything to show this is not fake relationship
5/ Application, white, for landlord (In the end, office don't need this document, so confused. :S )
6/ 2 passport size photo
7/ Health insurance card
